Some Forest Folk workshops involve sharp tools, heat, foraging, outdoor terrain or another higher-risk activity. When one of those workshops is booked, Jas will ask you to complete this form before the session starts. Please fill it in for the person taking part.

This records your understanding and consent. It does not replace the workshop safety briefing, and it does not exclude any rights that cannot legally be excluded.
